The SELLER, LuAnn
Rozek, hereby sells to the buyer __________________________, a male/female Goldendoodle puppy , for the amount of $______________.
The BUYER agrees that the puppy is being purchased as a pet and will not be used for breeding purposes. The BUYER agrees
to have the puppy spayed/neutered by the age of 1 year and send proof of such to the SELLER.
The
SELLER guarantees the above puppy to be healthy at the time of sale.
The puppy will
be checked by a licensed veterinarian. All appropriate shots and deworming for the puppy's age have been given and a
record of the vaccine history will be supplied. The puppy's dew claws have also been removed.
The
BUYER agrees to have the puppy checked by a licensed veterinarian within 7 days of taking possession of the puppy. If
the puppy is found to be other than healthy by the BUYER'S veterinarian, it shall be returned to the SELLER immediately, and
the purchase price will be refunded. Verification from the veterinarian's office will be required to return the puppy
for a refund.
I also offer a 2 year health
guarantee for any life altering genetic diseases. If within the first two years of the puppy/dog's life it is diagnosed
with: Hip Dysplasia with OFA or Pennhip Verification, along with copy of x-rays sent to Breeder's Vet, any debilitating
eye disorder diagnosed by a Veterinary Ophthalmologist; or any other life-altering genetic disorder, diagnosed by a qualified
Veterinarian, that requires treatment in order to preserve the dogs quality of life, the buyer would be entitled to a full
refund of the purchase price of the puppy. The Breeder has the right to have a 2nd opinion with our Veterinarian to
confirm the diagnosis. The guarantee is valid only if the following requirements are met.
1.
The BUYER agrees to maintain the puppy's health in good condition, and to provide yearly examinations, vaccinations, heartworm
test, and any other usual procedures necessary to assure good health.
2. There
is no indication that the dog has been physically abused.
3. The BUYER has
written documentation, x-rays, or any necessary paperwork from a licensed Veterinarian.
4.
The BUYER agrees to have the puppy spayed/neutered by the age of 1 year and provide a copy of spay/neuter certificate to the
SELLER.
The BUYER agrees that he will not sell this puppy or transfer ownership without the
SELLERS knowledge and written consent.
The BUYER agrees that if at any time he/she cannot take proper care of the puppy/dog,
the BUYER must contact the SELLER and the SELLER will have the first choice to either: (a) take the puppy back or (b) assist
the BUYER in finding a suitable home for the puppy.
The
SELLER does not assume any liability for any injury to the puppy after delivery.
